HOW TO USE THE DIAGNOSIS EDIT FEATURE

 

With TPS2000 you have the ability to set a diagnosis for one visit, or even one specific service if needed.  You may also, edit or change the permanent diagnosis of a patient’s claim from the service area of the program.

 

From the “Select a File to Open” menu choose Service and Payment Entry, use the Navigation Bar Icons, click on the . 

 

If this is the first time today that you have entered the services area, a list of your active, Patient Claims will popup.  If you have been working in services previously, the last patient claim for which you entered services will load.  You may select a different patient by clicking on [Select Patient Claim].  You may select from this list by scrolling or using the arrow keys to move up and down through the list, or you may click into either PAT CODE or NAME fields, and start typing.  If you know the patient’s code for whom you are ready to enter services, click on a number in the PAT CODE field and type in that code.  TPS 2000 will take you to that listing. If you know the patient’s name for whom you are ready to enter services, click on a name in the NAME field and type in all, or part of the patient’s name.  TPS 2000 will take you to that area of the list.   When you have found the correct patient, double click, hit enter or click on Select Record.

 


From the Services List shown on the left hand side of the screen, select the services that are applicable for this visit.  You may select from this list by scrolling or using the arrow keys to move up and down through the list, or you may click on the down arrow       next to Categ. and select a specific category of services from which to pick.

 

At the top of the screen, you will see the current diagnosis that has been entered on this patient’s claim. 

 

To Edit the diagnosis entry click on the [EDIT] button in the upper right hand corner of this screen.

 

The following screen will appear


You may ADD to this list of diagnosis entries.  You may DELETE any of the existing diagnosis entries.  You may REORDER the list of the current diagnosis entries.

 

ADD

Find the additional diagnosis entries in the list to the left and double click or click on [Add Item]. 

 

DELETE

Click once on the diagnosis that you wish to remove.  Then click on the Delete Icon on the navigation bar (7).  Repeat for any other diagnosis entries that you wish to remove.

 

REORDER THE ENTRIES

Click once on the diagnosis entry whose order you wish to change.  The number that is seen in the D.# column represents the order in which it will appear.  Change this number and hit enter, the diagnosis entries will renumber themselves accordingly.  For example, in the above example 839.21 Thoracic Subluxation appears as the third diagnosis.  To move this to the Primary or first diagnosis entry, change the 3 in the D.# column to 1 and hit enter.  Repeat for any other diagnosis entries that you wish to reorder.

 

Once you have made the changes that you need to the diagnosis entries, you next decide whether or not these changes are permanent or for this visit only.

 

At the bottom of the Diagnosis Edit screen you have several buttons.  Following is how these would be used.

 

Load Current Diagnosis From Claim

This button will load (or reload) the last saved diagnosis entry in this patient’s claim. 

 

Save These Diagnoses to Claim

This button will save the changed diagnosis list as the new permanent record.  This means, any service entries from NOW until another change is made, will be set to these diagnosis entries.

 

Apply These Diagnoses to the Current Service

This button applies this diagnosis list to the current service that you just entered. (PLEASE NOTE:  If you do not click on Save These Diagnoses to Claim, these entries will be for these current services only!).

 

Add More Services with These Diagnoses

This button will close the Edit box and take you back to the Services screen, ready to add more services with this same diagnosis setup. (PLEASE NOTE:  If you do not click on Save These Diagnoses to Claim, these entries will be for these current services only!).

 

Apply to Ledger

This button will bring up the following screen:

 

 

 

 

 

 


 Select the doctor(s) for whom you are editing these diagnosis codes.  You must highlight at least one doctor (even if there is only one doctor on this list) or no service lines will be corrected.

 

You have the choice of replacing only the services that are tied to EMPTY or No diagnosis entry, or you may, if you choose, replace ALL services in the ledger with this new set of diagnosis entries.

 

Close This View

This button will close the Edit box and take you back to services entry.

 

Let’s take an example of how this might be used.  Bob has been a patient for some time and is being treated for a low back or hip injury.  On Saturday, Bob hurt his left forearm playing baseball.  Since this visit is entirely dealing with the left forearm, none of the previous diagnosis entries will apply, nor will these diagnosis entries (dealing with the forearm) need to be added to the current diagnosis in Bob’s claim.  So, when he comes in for this visit, we pull up the Service/Payment screen, click on the [Edit] button and delete all of the current diagnosis entries.  Next, we select the diagnosis that is pertinent to this forearm injury.  Then we click on Add More Services with These Diagnoses this will take us back to the Service/Payment screen, and the diagnosis displayed will be those for this injury only.  Select the services and finish this cycle in the normal manner.  These forearm diagnosis entries will be saved in the Diagnosis History file, but the active diagnosis will remain unchanged.  When you bill, you will receive 2 HCFA forms, the forearm injury will be separate from the normal billing as they have a different diagnosis set.
