HOW TO ENTER YOUR
DIAGNOSIS LIST

You will need to setup a
master list of diagnostic codes for use in your practice. In this section each of the categories of
Diagnoses, Symptoms, and Complications will now be broken down into specific
Diagnoses, Symptoms, and Complications with their ICD codes. ICD stands for International Classification
of Diseases. ICD Codes is recommended
for use in all clinical settings but is required for reporting diagnoses and
disease to all US Public Health Service and Health Care Finance Administration
(HCFA) programs.

starts the Category name (type) will inhibit these symptom codes from being
available to print when billing carriers.

Note: The "D-“ that
starts the Category name (type) ensures that these complication codes will be
available to print when billing carriers.
Note: You may create several different categories
of Diagnoses, Symptoms, or Complications.
Note: The maximum number of characters allowed in
the description is 34.
Note: Please be sure that any Diagnosis you wish to
see on the HCFA form begins with "D-" . Currently, complications or symptoms (those
listed as "C" types or "S" types) do not print on the HCFA
form.
To add a new record click
on the “+” Plus sign on the navigation bar at the top of your screen. Now type in the ICD code for your new
entry. Use the arrow to the right to
choose the Diagnosis Type. Next type in
the Name for this ICD Code. Keep in mind
that a single ICD code could have many different designations. Type in the name you most commonly use for
this code. Remember, some of the other
designations will be automatically assumed based on the related ICD codes for
the claim.
The Notes section will be
for your use. You may choose to use this
feature or not. You may enter notes here that better describe or give an
additional description of the ICD-9 code.
For example, if you have
new staff who are somewhat new to a medical practice, you can enter in more
easily understood descriptions. Let's
say, the ICD-9 code is 739.1, and the description is, Cervical
Subluxation. Some new staff members may
not understand "cervical" or "subluxation". In the notes section, you could type something
like, "A partial dislocation of a vertebrae in the neck."
